An application for individual financial assistance from the Montana Lions Sight and Hearing Foundation requires several items of information. Among the items to be filled out are: a sponsoring lions club is recommended to submit the applicants name; an explanation of what the financial assistance will be used for; any other organizations requested help from and their response; health insurance coverage; financial statement; diagnosis and recommendation by your eye care professional, hearing or speech specialist.
